I am assuming you have hammered it down by hitting the small extension piece on the head for this purpose. If it still comes off then the best thing to do is force a metal piece (bent nail) etc down the side between the handle and the head as there is obviously a small gap allowing it to come loose. This can happen when the handle shrinks or expands
